Medical Check Ups

So, you have been admitted to KU, yeahh!!! What's next, besides preparing for your immigration documents, it is also imperative that you get your medical check ups done in Malaysia for 2 reasons: convenience and cost. 

In you pre-arrival package that KU will send to you, there will be two sheets of forms regarding health history and immunization. 

The yellow one will be the health history form where you just basically fill up your, well, health history to the best of your knowledge. Once you are done, just sign your signature at the bottom. 

It should look something like this.

The other one which requires more attention is the green form, which is the immunization screening records. Important immunizations to take before coming to the US are the following:

• Most of you should have a first shot of MMR when you were a kid, but KU wants two shots, so be sure to take another shot of MMR immunization before you come. 
• Take a shot of Meningococcal Meningnitis immunization. 

*Have your doctor sign for you at the green form.

*The TB immunization will be done by KU, so don't worry.

My advice is once you are already 100% sure that you wanna come to KU, do these medical checkups early, because you cant take two different type of immunization on the same day. So act quick!

THINGS TO BRING WITH YOU TO KU:

- A copy of your immunization records since your birth detailing all the immunizations you have had. (This should be a small book given to your family when you were given birth)

-The health history sheet. (the yellow form)

-The immunization records sheet. (the green form)

- If you have any surgeries before, it would be better to bring with you a copy of your doctor's letter. If you couldn't find it then it's okay. 

Now you are one step closer to coming to KU!!
